Referral Form

How to submit a referral form:

1. Click the ‘RCS Referral Form‘ to open the form in a new tab
2. Fill and complete the form with as much information as you are able
3. Save the form to your computer, and attach the document to an email directed towards the following;
   • Lansing – fax.lansing@realitycs.com
   • Charlotte – fax.charlotte@realitycs.com
   • Grand Rapids/Wyoming – fax.gr@realitycs.com
